MEMBERSHIP APPLICATION – Please follow these instructions:

1. Applicants must be at least 18 years of age.
2. You can only view the Application PDF file below.
3. Fill out the form – information will be inserted in the PDF file.
4. Submit the form.
5. You will be asked to sign and date a printed form in person.
6. Annual dues for members is $30 – collected when you sign.
7. Contact SWV for information about Student Membership for high school students ages 16 to 18 (dues $10).
